VERY IMPORTANT!

You will not be allowed to hunt unless each of the following documents is completed and given to your Huntmaster:

• Final and Unconditional Release of Liability
  (Must be signed by parent or legal guardian)
  

• Health History
  (One form for each person attending the hunt)

• Marksmanship Certification
